For your convenience, we accept Visa, MasterCard and Direct debit. Because we offer the best care at the most reasonable cost to our patients, payment is due upon the time the service is performed unless other arrangements have been made in advance. Please note that you are fully responsible for all fees charged by this office, regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies respond within four to six weeks. If your statement does not reflect your insurance payment within that time frame, contact our office. Any remaining balance after your insurance has paid is your responsibility. Full payment is required at the time of the service, and your insurance company will reimburse you directly. We can make arrangements for a monthly payment plan, such an arrangement will need to be implemented in advance of the procedure.
